An NBCH Minisite for Coalitions Working on Quality Improvement and Value-Based Purchasing
Improving Diabetes Care in Communities
Since March of 2005, NBCH and AHRQ have worked together to explore ways local employer-based coalitions could use information and technical assistance to improve the quality of diabetes care and prevention. Diabetes was selected as the focus of the initiative because there is clear evidence on best clinical practice, data to measure quality and cost are available and it is a disease that affects a large cross-section of the population.
AHRQ agreed to help three selected NBCH member communities with access to data, consultation on evidence and research, and technical assistance. The Lewin Group, Inc. was engaged to provide technical assistance to local groups following through on AHRQ information for work planning, implementation and tool development. The Greater Detroit Area Health Council, the MidAtlantic Business Group on Health and the Memphis Business Group on Health were chosen as pilot sites for the initiative.
NBCH Education and Research Committee (EAR) oversees the Collaborative
NBCH designated the EAR Committee to coordinate the initiative with NBCH members and staff. The committee has devoted a part of each meeting's agenda to talking with pilot site leaders, advising staff and distilling the information and experience gained into a format that will be useful to all NBCH members working to improve the quality and efficiency of local health care resources. Spreading Value Improvements
The EAR committee has collaborated with the Institute for Healthcare Improvement (IHI) for implementing methods for spreading successful projects, policies and tools. IHI has developed a "Framework for Spread" based on their experience in helping hospitals and clinical teams to learn and apply better ways for organizing and delivering care. The EAR committee adopted the Framework for this initiative and recommends that coalitions consider this model for spreading the use and impact of any projects undertaken to reach all possible users in the community. The mini-site is intended to be a vehicle for all to share their knowledge and tools and to network across communities and stakeholder groups to make measurable improvements. It contains a comprehensive collection of models, tools, site experience and links for the improvement of diabetes prevention and care at the community level.
