Population Health Seed Grant
The National Business
Coalition on Health and its partner organization Community Coalitions Health
Institute (NBCH/CCHI) are engaged in an ongoing partnership with the
Association of State and Territorial Health Officials (ASTHO) and the National
Association of County and City Health Officials (NACCHO). This is a multiyear collaboration supported
through a Cooperative Agreement from the Centers for Disease Control and
Prevention (CDC) to promote the development and sustainability of Community
Health Partnerships (CHP). CHP are
defined as multi- stakeholder collaborations that include leadership from
business led coalitions and public health organizations. Funding for the CHP Seed Grants was made
available through foundation and corporate partners of NBCH, including Merck, sanofi-aventis, and United Health Foundation.
The projects to be funded
under the Community Health Partnership Seed Grant Program must be initiatives
to promote population health. Summary
requirements include:
Developed
plan ready for implementation for a new or continuing initiative Collaborative
approach preferably with broad stakeholder involvement Measureable
outcomes related to population health improvement Use
of public health and other data such as eValue8 results used to identify
and plan for the initiative that is proposed Projects
may be related to health promotion or prevention addressing respiratory
conditions, diabetes or any public health topic linked to an indicator
from the United Health Foundation “America’s Health Rankings®” tool. Respiratory conditions include COPD,
asthma, clean air conditions or other identified issues.
