Fourteen diabetes seed grants were funded to promote innovative diabetes quality improvement and prevention programs driven by employer health coalitions at the community level. Funding was made available for this program through the support of Novo Nordisk as the founding sponsor, along with sanofi aventis, Merck, and Pfizer. This report examines the self reported results of the Diabetes Seed Grants completed in April 2009.
NBCH/CCHI Diabetes Seed Grant Final Report
In
2005, NBCH served as a subcontractor to Lewin and Associates for a
continuing, AHRQ-sponsored project to support three coalition led
initiatives in diabetes in an effort to inventory projects and identify
best practice models. The coalitions involved were the Mid-Atlantic
Business Group on Health, the Greater Detroit Area Health Council, and
the Memphis Business Group on Health.
CCHI's
first funded project- the eValue8 Research and Reporting Project -
focused on the need to better mine the eValue8 database and produce
common report templates for varying users.
